Continuum of Care Pre-Admission Unit

Our role is to ensure a smooth, safe, and well-coordinated journey for every patient—from pre-admission through to discharge and beyond.

Within the Pre-admission Unit, our team works closely with patients before their hospital stay to:

 

• Review your medical history and surgical needs
• Identify any risks or special care requirements
• Provide important information about your procedure and recovery
• Coordinate services such as allied health, discharge planning, and follow-up care

As part of this process, we also contact you directly by phone to review your medical history, clarify any details, and make sure you’re fully prepared for your upcoming procedure. This call gives us the opportunity to answer your questions, offer guidance and tailor your care to your individual needs.

We work in close partnership with your surgeon and anaesthetist to ensure that everything is in place for a safe and successful hospital experience—and a smooth transition home.
